What Is Scrupulosity?

During OCD Awareness Week, it’s important to highlight the many different subtypes of Obsessive-Compulsive Disorder (OCD). One lesser-known—but deeply distressing—form is called scrupulosity, sometimes referred to as religious OCD.

Scrupulosity involves an intense fear of sinning, offending a higher power, or violating religious or moral beliefs. For individuals living with this form of OCD, their intrusive thoughts often revolve around spiritual or ethical concerns—leading to a cycle of anxiety and compulsive behavior in search of relief or reassurance.


Common Obsessions in Scrupulosity

People experiencing scrupulosity may be overwhelmed by:

  • Fears that they have committed a sin

  • Intrusive thoughts about offending God or a higher power

  • Fears of eternal punishment (e.g., going to hell)

  • Doubts about their salvation or spiritual worthiness

  • Obsessions about being possessed

  • Intrusive uncertainty about religious beliefs or rituals

These obsessions are not chosen—they are intrusive, unwanted, and deeply distressing to the person experiencing them.


Typical Compulsions in Religious OCD

In an effort to reduce the anxiety caused by these thoughts, individuals often turn to compulsions such as:

  • Excessive confession (to clergy, friends, or even themselves)

  • Repeated reassurance-seeking from religious leaders or family

  • Purification rituals

  • Compulsive prayer repetition

  • Mentally repeating sacred texts or phrases

While these behaviors may seem like personal expressions of faith from the outside, the underlying motivation is fear, not devotion. And unfortunately, the more someone gives into compulsions, the stronger OCD tends to grow.
